Telehealth Consent

Draft for provider and legal review. Telehealth uses electronic communications to connect you with an independent licensed provider. It has benefits and limitations compared with in-person care.

What to expect

Your provider may review information asynchronously or request a video or telephone visit. Technical interruptions, incomplete information, or the need for an examination may require alternative care.

Voluntary participation

You may withdraw consent, but doing so may prevent completion of telehealth services. Your provider determines whether telehealth is appropriate for your circumstances.

Emergencies

Telehealth through this service is not for emergencies. Call 911 for urgent or life-threatening symptoms.
